Role Description

Rapid Recovery Center (RRC) is dedicated to providing effective care in an exceptional environment with a personal approach. As an RN case manager, you’ll be responsible for ensuring that each care plan is executed correctly with that personal touch.

At RRC, we believe that the best results are obtained when the entire care team works together, including nurses, therapy, physicians, hospitality, and culinary. You’ll embody this idea, working collaboratively with other health care disciplines to coordinate the best care plan for patients. You’ll also perform initial client assessments and coordinate the discharge of patients.

Rapid Recovery Center is changing the approach to post-acute care to add a personal touch and dignity back into it. As an RN case manager, you’ll be working to ensure that personal touch is there and ensure the best care is delivered at the same time.


Overall Responsibilities:

  • Performs patient assessments during the discharge process
  • Delivers personalized nursing care that meets or exceeds the health needs of each patient guest
  • Administers medications, performs treatments and procedures as ordered by the physician with the nurse scope in accordance with RRC polies and procedures
  • Participates in the care planning process with other health care disciplines and oversees implementation of the plan
  • Perform new admission assessments, receives and verifies new orders and treatments and enters the data into the electronic medical record
  • Documents assessments and care in compliance with standards of care and company policy using the computerized charting system
  • Encourages, delivers, and documents guest-specific wellness program treatments, and educates the guest about the available leisure activities
  • Maintains accurate and timely clinical documentation in all assigned areas
  • Ensure that the initial care plan is completed upon admission
  • Successfully supervises LPNs and CNAs as evidenced by collaborative teamwork on the shift
  • Demonstrates consistent management of nursing service problems and emergency situations
  • Creates and maintains a positive work environment and culture as reflected in staff satisfaction

What we’re looking for

  • Graduated from an accredited school of nursing; BS in nursing is preferred
  • Valid RN state licensure from an accredited college
  • Current CPR certification
  • Three to five years of experience as an RN in a high volume, fast paced patient care setting
  • At least 1 year serving as a Case Manager or performing patient discharge assessments
  • Excellent communication skills with a hospitality mindset
  • Proficient with computers and documentation and ability to use automated processes
