Role Description

The Director of Social Services is responsible for directing the social service program in a Rapid Recovery Center (RRC) ensuring a positive overall experience for the patient. 


Overall Responsibilites: 

  • Participate in care/discharge planning within first 24 hours according to RRC service standards; develop and implement social care plans and patient assessments
  • Provide support and assistance to patient and family for transition care plan
  • Provide information to patient and families as to Medicare and other financial assistance programs available to the patient
  • Interview patient and/or family members, as necessary, to obtain social history
  • Assure that social service progress notes are informative and descriptive of the services provided and of the patient response to the service

What we’re looking for

  • Bachelor’s degree in Social Work
  • Must be licensed by the state
  • One years of social work experience preferably in a transitional care or long term care setting
  • Excellent communication skills with a hospitality mindset
