Role Description

Rapid Recovery Center is dedicated to providing effective care in an exceptional environment with a personal approach. As an RRC Director of Social Services, you’ll be able to take a personal interest in our patients to make that happen.

A social service program is an essential part of post-acute care. You’ll be responsible for directing that social service program. You’ll also be working directly with patients, and be an instrumental part of creating a positive patient experience.

Rapid Recovery Center is changing the approach to post-acute care to add a personal touch and dignity back into it. As an RRC social work, you’ll be working to ensure that personal touch and dignity is part of everything we do.


Overall Responsibilities:

  • Plan, develop, organize, implement, evaluate and direct the social service programs for the RRC
  • Participate in care/discharge planning within first 24 hours according to RRC service standards; develop and implement social care plans and patient assessments
  • Provide support and assistance to patient and family for transition care plan
  • Provide information to patient and families as to Medicare and other financial assistance programs available to the patient
  • Review and develop a plan of correction for social service deficiencies noted during survey inspections and provide a written copy of the correction plan to the Administrator
  • Interview patient and/or family members, as necessary, to obtain social history
  • Assure that social service progress notes are informative and descriptive of the services provided and of the patient response to the service

What we’re looking for

  • Bachelor's degree in Social Work, Master's degree preferred
  • Three years of social work experience preferably in a transitional care or long term care setting
  • Excellent communication skills with a hospitality mindset
  • Strong leadership experience
